What is Ovulation?

Ovulation is the release of a mature egg from one of your ovaries into the fallopian tube. Once released, the egg remains viable and open to fertilization for approximately 12 to 24 hours. If fertilization does not occur within this period, the egg disintegrates and is absorbed, and your period will begin about 14 days later.

When Does Ovulation Occur?

In a typical 28-day cycle, ovulation usually occurs around Day 14. However, menstrual cycles vary widely (from 21 to 35 days). As a general rule, ovulation occurs 14 days before the start of your next period. For example, if your average cycle length is 30 days, you will likely ovulate around Day 16.

The Mathematics of the Fertile Window

If the egg is only viable for 12-24 hours, why is the fertile window 6 days long? The answer lies in the longevity of sperm inside the female reproductive tract.

Fertile Window Equation:
Sperm lifespan (up to 5 days) + Egg lifespan (1 day) = 6 Days of Fertility

Because sperm can survive in high-quality cervical fluid for up to 5 days, intercourse that occurs 5 days before ovulation can lead to pregnancy. Your peak fertile days are the two days immediately preceding ovulation and the day of ovulation itself.

The LH Surge and Ovulation Timeline

  • LH Surge Begins: The pituitary gland releases a surge of LH, which travels through the bloodstream to the ovaries. This signal starts the process of releasing the dominant egg.
  • Peak Surge: Occurs in the bloodstream first, then appears in urine about 12 hours later. This is when an over-the-counter ovulation test will show a positive result.
  • Ovulation Occurs: The egg is released from the follicle approximately 24 to 36 hours after the LH surge is detected in urine.

What is Anovulation?

Anovulation occurs when an egg is not released during a menstrual cycle. While an occasional anovulatory cycle is normal (especially during times of high stress or illness), chronic anovulation is a primary cause of fertility difficulties. Common causes include:

  • PCOS (Polycystic Ovary Syndrome): A hormonal imbalance causing multiple small follicles to grow in the ovaries without maturing and releasing.
  • Thyroid Imbalance: An overactive or underactive thyroid interferes with the hormones governing ovulation.
  • Hyperprolactinemia: Excess production of prolactin (the breastfeeding hormone) halts ovulation.
  • Low BMI or Over-exercising: Suppresses the hypothalamic-pituitary axis, causing periods to stop entirely.
Note: Anovulatory cycles can still produce bleeding that looks like a normal period. Regular tracking of symptoms like cervical mucus and basal body temperature is the best way to confirm you are ovulating.
